Bar-Mitzvah Club: Sunday from 10:30-12:30 or Tuesday from 4-6PM

Medical Emergency

As parent/guardian, I authorize Chabad of Hewlett representatives to seek medical treatment for my child(ren) in case of an emergency. I understand that efforts will be made to contact me if time permits, and I agree to cover all medical expenses.

Media Permission

I grant permission for my child(ren)'s photos/videos to be used in promotional materials, newsletters, local newspapers, and the Chabad of Hewlett Website

Program Pricing:

$650 for the first child + $200 household security fee.
$625 for the second child.
$600 for each additional child.
$500 additional for Sunday & Tuesday.
